After working in a Psychological Hospital, processing the insurance billing for over a year, Robert was asked to give some additional help to a medical assistant working in another area of the hospital.  While chatting with some of the staff in the office, Robert couldn’t help but notice a change in their behavior when Dr. Salazar came into the office.  They seemed to have a level of disrespect and businesslike manner towards the Doctor.  Their demeanor countered their previous playful bantering exhibited toward the other doctors who had come into the office earlier.
Robert watched as Dr. Salazar ordered one of his nurses to find all of his patients and have them stand against the wall outside of his small intake room.  Robert noticed that Dr. Salazar spent between 5 to 7 minutes with each patient, herding them in and out of the tiny room, with little disregard to their concerns or their requests to discuss their issues with him further.  Dr. Salazar had managed to see all of his patients, make notes and write scripts within an hour.  After he left, Robert overheard the nurses gossiping about how Dr. Salazar was the “candy man” because he handed out medications like it was candy, and then billed the insurance company for 45 minutes for each patient.
What is Robert legally supposed to do with this information about Dr. Salazar, if any?
Has there been a violation legally or ethically?  If so, what?
If Dr. Salazar’s actions are called into question and Robert does not speak up, can he be held liable? Why or why not?

Cecilia had returned to school to obtain a degree in Medical Office Administration because she felt passionate about helping people. She found a job in a mid-size clinic and realized there were quite a few patients who were not only economically disadvantaged but also, could not speak English. She, and other staff members found themselves often frustrated at how they were struggling to breach the language barrier and receive all of the correct information from the patients.
As Cecilia listened intently to one elderly woman who spoke only Spanish, try to describe her medications, she became worried that her best efforts to decode what the woman was saying may cause some problems for her down the road. Cecilia documented what she could and sent the woman in to see the doctor. The doctor, who could speak Spanish as well, came out after examining the woman, complaining that she hadn’t understood how to properly take her medication and therefore, had been taking additional medications along with her prescriptions to relieve her ongoing symptoms.
The doctor chastised the staff for not doing their jobs and putting patients lives at risk. He demanded they get their acts together and come up with a plan to fix all of the obvious miscommunication that was going on in the office.

As much as Barbara loved her job as a Medical Office Assistant, she knew she eventually would want to move on from there and probably work in Billing and Reimbursement. She helped Carol out quite a bit, who was a biller and coder, and thought it was a lot like having to try and solve a puzzle. She also felt excited that Carol was willing to show her what the job entailed and trusted her with a few tasks.
Carol stated, “No matter what, you need to appeal every denial that we get from the insurance company. Look at it this way, if the doctor doesn’t get paid, you don’t get paid.” In addition, Carol instructed Barbara on her on how she would handle the next set of tasks. She stated, “It doesn’t matter what reports you run first. Just so you get it done. We’ll worry about the details later.” When Barbara stopped to question the office procedures, Carol let be known that she didn’t care what she had been told, but that her instructions were the way to do things.

Unit 5 Assignment 1 – Case Study #5: Various Diseases and Treatment Processes
Instructions
Joann was hired in a medical office and medical office administrator, part of the job description required her to answer the phones and be on the front lines when dealing with the patients.  She needed to quickly assess if the patient needed a same day appointment, be scheduled for an appointment in the near future, or go straight to the emergency room based on the information the patient provided over the phone.  This meant she needed to have a proficient working knowledge of a wide variety of diseases, their symptoms, their level of seriousness, and the treatment associated with them.  She understood that if she gave wrong information to a patient, and scheduled an appointment with the doctor, instead of directing them for a more immediate intervention, the time lapse could result in serious consequences.

Jonathan had spent his weekends and summers working at Pizza Hut throughout his teen years, as well as, in college. He had been mentored by a boss he greatly admired, who taught him the value of hiring a diverse workforce. Over those formative years, Jonathan had many opportunities to work with and befriend, people who had a variety of disabilities. After years of hard work, followed by a series of promotions, Jonathan had become a Medical Office Administrator and decided to pattern his leadership style after his boss at Pizza Hut. He wanted to make sure that his office represented “real” people who could reach out to the patients who would be coming into his office.
Susan, a 26-year-old intellectually disabled woman, had worked in Jonathan’s office for a year. She had become a valuable asset to the staff as a clerical assistant with her duties of, greeting the patients, sorting the mail, running errands, and sometimes answering the phones when the office became busy. Marcus, a Medical Administrative Assistant, liked Susan well enough but found himself becoming frustrated with her because he felt like she was underfoot and slowed him down. In addition, he took his job seriously and believed any mistakes Susan made, reflected poorly on him.
During a particularly busy afternoon, the waiting room was full, and the phones were constantly ringing. Trying to keep his composure and not let the hectic situation get the best of him, Marcus finally had had enough of trying to delegate tasks to Susan and deal with restless patients. He shouted across the office into the back area, “I could use a little help in here! Can I get someone to help me who’s working with a full deck!?” Susan feeling embarrassed, began to apologize to Marcus for “messing everything up.”
Hearing this public exchange, Jonathan could hardly contain his own anger and called Marcus into his office.
